How to Complete the Nomination Form
Thank you for nominating individuals whose breakthrough research transformed the fields of astrophysics, nanoscience or neuroscience.

Please read the following instructions before completing the nomination form. 

Each nomination consists of five sections: nominee information, accomplishments, publications, references and nominator contact. At the end, you can preview your nomination. 

Please note that the nomination form must be completed in English and in a single session; you will not be able to save your progress before submitting. Before you begin, review the submission requirements below to ensure you have all relevant information.

We encourage you to prepare the Brief and Detailed Descriptions of Breakthrough, and Curriculum Vitae sections in a separate document. Next, paste it into the nomination form. Sections with character limits count commas and spaces toward the total.

  • Shared Prize: Indicate if the nomination is for a shared accomplishment. If it is, you must write separate nominations for each of the nominees. Up to five individuals may be nominated for a shared prize.

2. Accomplishments:

  • Brief Description of Breakthrough (Max. 250 characters, including commas and spaces): This may be used as the official quote when saying why the person(s) was awarded the prize. See examples by visiting www.kavliprize.org. 
     
  • Detailed Description of Breakthrough for The Kavli Prize (Max. 2500 characters, including commas and spaces): Describe the nominee's breakthrough discovery that justifies the awarding of The Kavli Prize. Collaborative work must be acknowledged in this statement if it is listed in Publications. In such cases, it is important to distinguish the contributions of the nominee from the collaborators.
     
  • Curriculum Vitae (Max. 2500 characters, including commas and spaces): Provide the following nominee information: Undergraduate degrees, institutions, and year conferred; graduate degrees, institutions, and year conferred; professional positions, institutions, and years held, listed chronologically (identify all positions and dates held at the same institution); post-doctoral fellowships (include years from-to); significant memberships; major awards (identify organizations that sponsored awards) and award years.

3. Publications:
List up to five of the nominee’s papers most relevant to the breakthrough discovery. For joint publications, the authors should be listed in the same order as in the article originally published. Collaborative work must be acknowledged in the Detailed Description of Breakthrough for The Kavli Prize.
